This pattern usually means the approval prompt is not reaching the expected Authenticator session, or the app cannot complete the prompt flow.
Try these steps in order:
- Check whether the prompt is going to an older device If Microsoft Authenticator was previously active on another phone, the push notification may still be sent there instead of the current device. If the old phone is still available, open Authenticator there and approve the sign-in. Then remove the old device from the Authenticator setup so future prompts stop going there.
- Make sure Authenticator is unlocked and notifications are enabled If the app is locked, the entry box might not appear. Silent or disabled notifications can also prevent the approval prompt from showing. Open Authenticator directly and confirm notifications are allowed for the app.
- Open the app directly when the sign-in is waiting Even if the popup does not appear automatically, open Microsoft Authenticator manually and check whether the account is listed and the app is responsive.
- Update Microsoft Authenticator An outdated app version can cause authentication failures. Install the latest version from the device app store.
- Check connectivity and device time The prompt flow can fail if the phone has network issues. Switch between Wi‑Fi and mobile data and try again. Also make sure the device date and time are correct and set automatically. If Authenticator notifications always appear expired, Microsoft recommends reconfiguring the system clock to automatic and then restarting the device.
- Retry after a short wait Temporary Authenticator issues can cause sign-in failures. Wait a few moments and try again.
- Use another verification method if available If the number-matching prompt still does not appear, use a different verification method such as SMS, if that option is available.
- If this is a work account and the issue continues, contact the IT admin Since the account was previously fixed by rescanning a QR code and creating a new Authenticator registration, the current registration may need to be reset again by the organization. If all steps above fail, send logs from Authenticator using Send feedback from the app menu, then reach out to the IT admin or helpdesk.
Also, the 6-digit code and the number-matching approval prompt are different flows. The 6-digit code changing every 30 seconds is expected, and some sign-ins require approving the number prompt instead of entering that code.
