Artifact Signing Identity Validation Failed - No Reason Provided

I am using Artifact Signing Identity Validation. I received requests for additional documents and provided them. On my last submission I received this notice that it failed: We've reviewed your appeal for verification. Based on the information you have provided to date; we have determined that your organization does not currently meet the requirements to pass verification. As this is your third appeal, we have closed your application. Thank you, Vetting Operations Support

I understand it failed, but could someone help me get feedback on why my last document failed? It's my first time requesting for Identity Validation, so my first documents I received feedback on (ie. the document needs to be within the past 12 mo), but with my last submission I received no feedback. Could someone help me with this so I can resubmit with the correct document? Thanks!
