This is an MFA lockout loop. The Authenticator app cannot generate or approve sign-in if the account is no longer available in the app or the app requires the same MFA method to get back in.

Try these recovery paths in order:

    1. If the account was backed up in Authenticator, try restoring it. If restore succeeds but the account shows Action required, open the account in Authenticator, tap the account tile, sign in, and complete the extra verification.
    2. If the app opens but sign-in fails, make sure Authenticator is unlocked, notifications are enabled, the app is updated, the device has internet access, and the device date/time is correct. Then try again.
    3. If the app shows Something went wrong. [4s8qz], wait a few moments, retry, update Authenticator, restart the device, and if it is a work or school device also update Company Portal and confirm the device is compliant.
    4. If the account is personal and the backup account cannot be accessed, support agents cannot restore the Authenticator backup. Use the Microsoft account sign-in helper to regain access to the backup account. If restore is not possible for any other reason, add the accounts again.
    5. If the account is a work or school account and there is no other sign-in method available, contact the IT admin or helpdesk. They can reset MFA settings or require re-registration of authentication methods.
    6. If this is an admin account and no other admin can access the tenant, the supported path is to contact Microsoft Support for tenant admin recovery / MFA reset.

If unexpected sign-in prompts appear, select Deny for any request that was not initiated.
