Artifact Signing — organization identity validation fails, business document upload step never appears

I'm trying to complete organization identity validation for Artifact Signing (Public certificate type, US LLC).

The verification flow only presents personal identity verification — face and government ID via Microsoft Authenticator / AU10TIX. It never shows a step to upload business documents. After the personal ID step completes, the validation returns to Azure with status "Failed" and this error:

"Please provide additional document(s) listed below dated within the past 12 months to verify your organization. Formation/incorporation records, franchise or agency appointment letters, government-issued licenses, registration certificates, or tax filing records."

So documents are being requested through a step that was never presented.

What I have already checked:

  • No verification email arrived at the primary address, including spam, so this isn't an expired link
  • This failed on the first attempt, not a retry
  • Formation records, state registration certificates and tax registration records are all ready to submit as PDFs, dated within the past 12 months

Question 5976464 reports what looks like the same behaviour.

How do I get the business document upload step to appear, or can someone accept the documents directly to complete the validation? I'd prefer not to delete and recreate the request, since document attempts are limited to three.
