Artifact Signing organization identity validation stuck on "Action Required" Verified ID presentation reports success but never registers

An organization identity validation was submitted on 2026-08-23 and the portal still shows
Action Required, with no sub-status, no date and no action link on the row.

What happens, every time:
- The "Action required: Complete your Artifact Signing identity validation" email link opens
credentials.microsoft.com, which shows a QR code for presenting a Verified ID. - The Verified ID was issued successfully by AU10TIX at 14:23:55 on 2026-08-23 and appears under Verified IDs in Microsoft Authenticator.
- Scanning the QR runs the face check, then Authenticator shows a green tick and "Done, finishing up…", then the app closes itself to the home screen.
- The page shows "Processing…", then returns to a fresh QR code.
- The portal row stays Action Required. Nothing anywhere reports a failure.
Already ruled out — the presentation was retried after each of these, with a freshly minted QR every
time:
- Force-stopping every instance of Microsoft Authenticator before scanning (the remedy suggested on similar threads), on the current app build.
- A different desktop browser (Vivaldi instead of Brave), and clearing the page's cache.
- No VPN on either device: the desktop browser does not route through a VPN, and the VPN on the phone was
disconnected.
- A different network path for the phone, with Wi-Fi disconnected.
One earlier attempt returned Error Code: notFound — "Request with requestId … not found" — which was a
QR that had gone stale; every attempt since has used a QR minted seconds before scanning, and those end in
the silent failure above rather than an error.
The ask: please read the backend traces for the identity validation ID and request ID above and say
what the presentation is failing on, or reset the validation so it can be presented again. The credential
presentation window on that page expires 2026-09-22.

2026-08-25T23:45:39.4+00:00 Following the guidance to delete a stuck request and create a new one, I deleted the original identity validation and created a fresh one on 2026-08-23. There were no certificate profiles on the account, so nothing was linked to the deleted request. The replacement behaved identically — Authenticator ran the face check, showed a green tick and "Done, finishing up…", the page showed "Processing…" and then returned a fresh QR, and the row stayed Action Required. As of 2026-08-25 that row reads Failed. There is no sub-status, no reason and no notification explaining it, and Renew is disabled.
One measurement that may help diagnosis: the Verified ID card's own Activity log in Authenticator records no presentation events for any of the attempts — only the AU10TIX issuance (2026-08-23, 14:23:55, "Result: Successful") and a single "You shared: Signed in (self-issued.me)" entry — despite the app showing a green tick and "Done, finishing up…" on every attempt. The credential share appears never to have completed from the wallet, while the sign-in half of the handshake did.
