    Dear Camille Warrington,

I’m sorry this form and its collected responses have suddenly disappeared. Since it is not under Deleted forms, it may be stored under another account, collection, or Microsoft 365 group rather than being deleted.

Please try the options below and check whether the issue continues:

    • Sign out of Forms, open a private browser window, and sign in using the exact account that originally created the form. Then check All my forms, Recent, and any Collections.
    • If the form was created in Teams, SharePoint, or later moved to a group, open My groups in Forms and check each relevant group. A moved form no longer appears with personal forms.
    • Ask any co-author to check whether the form still appears for them and, if so, send you a new collaboration link.
    • Search OneDrive or SharePoint for an Excel workbook containing the responses, especially if Open in Excel was previously used. This may preserve the collected data even if the form is unavailable.

Note: If this is a work or school account, please contact your Microsoft 365 administrator so they can check whether the account, license, or form ownership has changed

Please let me know how it goes after trying these steps, and I'll be glad to continue looking into it with you.
