The account is stuck in a verification loop because Microsoft Authenticator is the only (or primary) sign-in method and it is being required to sign in to itself.

Use one of these approaches, depending on the type of account and what access still exists:

    1. Use an alternative verification method (if configured)
    • When prompted to approve the sign-in in Authenticator, choose “I don’t have my Microsoft Authenticator app” or similar.
    • Select another method such as SMS code, phone call, or email code if they are listed.
    • Complete sign-in using that method, then:
      • Add the account back into Microsoft Authenticator on the new phone.
      • Optionally add more backup methods so this loop cannot happen again.
    1. If this is a work or school account and no other method works
    • Contact the organization’s IT admin / Help desk and ask them to reset or re-register multifactor authentication (MFA) for the account in Microsoft Entra.
    • After the admin resets MFA, sign in again and set up Authenticator from scratch on the new phone.
    1. If this is a personal Microsoft account and no method works
    • Use the account recovery and password reset options (for example, via backup email or phone) to regain access.
    • Once signed in on a browser, reconfigure Microsoft Authenticator on the new device.

If the message shown is “Authentication did not complete”, also check:

    1. Authenticator is unlocked on the phone.
    2. Notifications are enabled for Authenticator.
    3. The app is updated to the latest version.
    4. The phone has a working internet connection and correct date/time.

If none of the above paths are available and this is a business account where the user is the only admin, Microsoft’s Data Protection team must be engaged via Microsoft Support to verify identity and restore access.
