My MFA is out of sync

I got a new phone a few months ago, and now my MFA doesn't work. I have a Azure subscription and My MFA does not work. I can log into the default directory but when I go to switch the directory with all my work, it fails on the MFA.
I have authenticator set up, and I see the account, but the code is never right and I never get the push notifications to approve.
