Azure Trusted Signing identity validation fails requesting documents but no upload option available

I'm setting up Azure Trusted Signing and the identity validation process immediately fails with the message:
"Please provide additional document(s) dated within the past 12 months to verify your association with the organization. Examples of verification documents are as follows:"
However, the status goes directly to "Failed" without ever showing an upload option in the Azure portal. I have tried creating multiple identity validation requests and the same issue occurs each time.
