12/21/18 - An Outlook.com engineer has confirmed that they are currently waiting for deployment of the fix for all affected users.

03/27/18 - Apple has released iOS 12.2 that fixes the sync issue with iOS devices for Outlook.com Contacts. (FIX: Unsync / Re-sync contacts)

Some users above state that 12.2 has not fixed their Outlook.com contact and that some contacts still have the incorrect date.
This might be due to the fact that a contact which was displaying the incorrect date on iOS for example, was edited and saved from an iOS device that altered the date on Outlook.com to the incorrect date.
I had a contact who's birthday is 10/23, but recently forgot I updated his home address, so when it saved back to Outlook.com, it saved the new address and the incorrect 10/24 birthday.

Below are the steps and actions that make this problem easy to reproduce:

It seems like the break is on how Outlook.com and iPhone synchronize this information. It's also strange the Outlook.com Contact View Mode shows one date, while Contact Edit Mode shows a different date than the displayed one.
These steps are easy to reproduce on my end on both my account and the new account created.
Some people state that since iOS12 that synchronization of Anniversaries are also affected. I did not try to validate those steps in this testing.

I am amazed you were actually able to communicate directly with an Outlook.com engineer and that this person took ownership of the issue. I am even more amazed that you have been told Outlook.com is working with Apple.

A "fix" for users is to de-select the iOS/OSX birthdays toggle in Calendar and select only the Outlook.com birthday calendar. That way at least you will only see the correct birthdays for contacts. Add a manual note for each contact that has a birthday. Ugly, but better than having a calendar populated with incorrect birthday dates.
Actually, it was Apple Support that was helping me through the issue - unfortunately it does not appear to be fully "fixed" in iOS 12.2 and not at all in OSX 14.4.4..
As for the fix -that was my workaround. Good to know that it is valid (though ugly)!
It's interesting you say that the issue has been around for years. I don't doubt that, but in my case, I have been syncing with Office 365 for 5+ years. Nothing has changed at all in my setup. This issue appeared for me between Dec 2018 and Feb 2019. So from my perspective, something must have changed.

As I am sure you are well aware, there are numerous posts/blogs/forums about this issue where a user tries to get help from Apple support or Outlook.com support only to be told to "Check your time zones, etc." Both companies should have this issue flagged so if anyone reaches out and says "Outlook.com...Birthdays shift......Apple......macbook/ipad/iphone", support staff will at least be informed it is a known issue.
Unbelievable that something as simple as a birthday date can't be interpreted the same way by two multibillion dollar companies.

It fixes the issue for me, make sure to have iOS 12.2 installed as I am pretty sure that it is part of the fix
I'm still seeing some weird things after updating to 12.2 - if I edit a date on iOS, it updates Exchange to one day prior! I had removed all contacts from iOS and then re-synced. I edited an "Anniversary" date and that made an incorrect update to Exchange. Have you seen something similar?
