Sounds like its the graphics card driver causing the issue. This isn't the expected behavior in windows. Graphics cards and shell programs can cause this issue.

every time I copy or paste & the dialog needs me to answer an overwrite, etc... the focus of the window doesn't come to the foreground of all the open windows.
VERY annoying.

2015-06-13T21:48:22+00:00 I'm having an issue with copy/move files dialog not appearing at all anymore. This happened on both my laptop and my desktop at the same time a few weeks ago after a Windows update. It was after a twelve updates in an update.
I've been looking to see if others have posted about this issue, and this is as close as I can find.
How do I get my file dialog back?
I want to see my file move or copy progress dialog! Why did Microsoft take it away?
Please help.

I wouldn't have believed it, but after updating my graphics drivers (GeForce 980), the issue seems to have been resolved.
The copy dialog appears on top as expected now.
Still, this seems like a situation where 3rd party settings should not have an effect like this on core OS functions and Microsoft should ensure primary control over it.

2015-06-01T19:01:44+00:00 This step backwards in Window 8.1 is much bigger than they admit. It started on my PC, most likely after an NVIDIA driver update. I do not have Chrome installed, so that's not the problem. This change is very annoying because the thumb drive, etc. may have already been removed by the time you see the subtle flashing frame behind the File Explorer button. By then you wonder which device's contents were not copied. It should be an easy no-brainer fix for Microsoft to make. This simple-to-fix problem will only become more of a problem as time progresses.
