How do I remove the following sentence from my Microsoft Survey "when you submit this form, it will not automatically collect your details like name and email address unless you provide it yourself"?

I agree with the statement by others....We're trying to use this as a way for staff to submit feedback.

But not all staff have a company account, so we have to choose the "Anonymous" option which adds the confusing "When you submit this form, it will not automatically collect your details like name and email address unless you provide it yourself." statement.

We want people to add their name and contact details, so we can contact them about their feedback.

It would be great if this statement can be optional.

Please take this statement out so that our organization can use this anonymous quiz without confusing everyone that we would like to share the form/quiz with, it is very unncessary!

Based on your description, the sentence "When you submit this form, it will not automatically collect your details like name and email address unless you provide it yourself" appears in Microsoft Forms when the form is set to allow Anyone can respond (anonymous response, doesn't require sign-in). This message is automatically displayed to inform respondents that their identity will not be collected unless they explicitly provide it. 

Therefore, you need to change the form settings so that it does collect respondent information and that sentence will be disappeared, here are steps to do: 

1.Open the form in Microsoft Forms. 

2.Select “Settings icon". 

3.Under “Who can fill out this form”, choose: “Only people in my organization can respond”. 

4.Then check the box for: “Record name”. 

This will make the form non-anonymous, and the message will change or disappear because the form will now collect the respondent’s name and email automatically.

The differences between 2 options: 

With "Anyone can respond": 

With “Only people in my organization can respond”: 

Besides that, you can choose "Specific people in your organization can respond" to hide the sentence, but this option requires you to enter the specific email addresses of the people you want to submit the form.

Kindly notes: 

• If the form is shared externally (outside the organization), Microsoft Forms cannot automatically collect identity information, so the message will remain. 
• This behavior is by design to ensure transparency and privacy compliance.
