I understand why this is concerning, especially since you have already contacted Support and were told to use an “account takeover form” that you cannot locate.
Could you please share the exact suspension or lock message you see, or post a screenshot after hiding your email address, phone number, case number, and any other personal information? In particular, please check whether the message includes a link beginning with aka.ms/. If it does, use that specific link to submit the account reinstatement request, as it is associated with the type of restriction placed on the account.
If no such link appears, start with the Microsoft account Sign-in Helper. It can identify the account issue and direct you to the available recovery or support option.
Please note that this is a user-to-user support forum. Moderators and contributors, including external Microsoft employees, cannot access account records, verify ownership evidence, or manually restore an account. For security and privacy reasons, Support agents also cannot directly change account details or bypass the official verification process.
