Title: Microsoft 365 Copilot used the wrong Word document during a document-specific review

Issue description:

On 1 September 2026, I used Microsoft 365 Copilot on the web and Copilot embedded in Word to review audit responses in the document “Phoebus Internal Audit_IA-12968_ICON Audit Response Template_RAuty_OT rev.docx.”

Although this was the document being reviewed, Copilot answered using content from a different file named “IA-12968_Audit_Report_with_Team_Response_Sections_QC.docx.” That second document was an earlier generated editable copy of the audit report and did not contain the completed team responses.

As a result, Copilot repeatedly stated that responses were missing or incomplete, even though the responses were present in the correct open document. This led to incorrect review conclusions, unnecessary rework and a significant loss of time during a time-sensitive GCP audit-response review.

Several IA-12968 files with similar names existed in SharePoint/OneDrive. However, the exact document being reviewed was open in Word, and the request concerned that document specifically.

Expected behaviour:

When Copilot is used within an open Word document and the request explicitly concerns that document, Copilot should use the open document as the primary source. If another similarly named file is being used, Copilot should identify the filename clearly before providing conclusions. Copilot should not state that content is missing without first verifying the exact active document.

Actual behaviour:

Copilot grounded its answers in a different, similarly named document and presented conclusions as if they applied to the open document.

Business impact:

The issue affected preparation of responses to a Major internal GCP audit finding, created incorrect review feedback and required substantial rework.

Requested investigation:

Please investigate:

  • Why Copilot selected a different file from the document open in Word;
  • Whether document context was lost between Copilot on the web and Copilot in Word;
  • Whether similarly named SharePoint/OneDrive documents caused incorrect grounding;
  • Whether the response can show the exact source filename used before making document-specific conclusions; and
  • Whether diagnostic logs can identify which file was retrieved for each affected prompt.
