Forms connection to workbook lost- Please Help!!

I accidentally disconnected my form responses from my customized workbook and reconnected to a new "blank" workbook. The previous workbook was customized with complicated formatting, linked in Zapier to my CRM and shared appropriately.

How can I reconnect the form responses to the original workbook?? I need the form responses to populate in the formatted workbook. Please help!!

    Hi @Jenn Lo

    Unfortunately, Microsoft Forms does not provide a way to reattach a form to a previously connected response workbook. Once the form has been disconnected and synced to a new workbook, Forms treats the new workbook as the active response workbook.

    Since you still have your original customized workbook, I understand why you want to keep using it, especially because it is already formatted, shared, and connected to Zapier and your CRM. However, there is no supported Reconnect option in Forms that allows you to select that existing workbook as the response destination.

    Here are 2 option you may consider:

    1.Keep the new Forms-connected workbook and update your integrations

    This is the simplest approach:

    • Open the form.
    • Go to Responses.
    • Open the newly connected workbook.
    • Update your Zapier connection to use the new workbook.
    • Reconfigure any CRM integrations, sharing permissions, formulas, or reports that depend on the old workbook.

    2.Use Power Automate to write responses to your original workbook

    If preserving your original workbook is important because of its formatting, formulas, CRM integration, and reporting setup, this may be the better workaround.

    You can create a Power Automate flow. This allows the form responses to continue arriving automatically while keeping your original workbook as the destination for the data. Please refer to How to Sync Microsoft Forms to Excel
