Auto-grading enabled
The issue occurs only for a few respondents, while the majority receive the correct score for the same question.
We verified that the correct answer is still marked correctly in the editor.
The issue is not consistent and cannot be reproduced for every submission.

Question:

Which university does this logo belong to?

Correct answer configured:

University (Option 1)

Observed behavior:

Some respondents selected Option 1 (University) but were awarded 0 points, whereas other respondents selecting the exact same option received full marks.

What we have already checked

Verified that the correct answer is configured correctly.
Verified that the quiz was fully configured before participants started responding.
No edits were made to this particular question after responses began.
Tested the quiz ourselves, and it scores correctly in most cases.

Since this quiz was used at a live student event, we'd appreciate your help in identifying the cause of this inconsistent grading behavior and whether there is any known issue or workaround.

Regarding your issue, may I kindly ask a few questions to help understand it better.

Is the incorrect grading limited to one question, or does it affect other questions in the quiz?
How many respondents experienced the issue out of the total number of submissions?
Did the affected respondents use the same or similar browsers, devices, or operating systems?
Could you provide screenshots showing:
- The question settings with the correct answer marked.
- An affected submission where the correct option was selected but no points were awarded.
Does the same behavior occur when you manually recreate the question in a new quiz?
In the Excel export, is the answer recorded for the affected respondents identical to the correct answer configured in the quiz?

1- This occurred to 2 questions.
2- Approx 10-15 students experienced, out of the total 145 submissions (total of all 5 quizzes)
3 - not sure

5 - No, we did not try this as it was a two day live event and we did not want to make any changes to the submission result.
6- We do not see any glitch in the excel report.

As the issue affected only some submissions and no problem is visible in the Excel report. I recommend contacting Microsoft Support so their team can review the Microsoft Forms service logs and investigate further.

A Microsoft technical support engineer can investigate the situation, review the relevant service information and backend configurations, and conduct a remote session if necessary. If the issue requires further attention, the support engineer can escalate the case to a specialized team for deeper analysis.

Before submitting the ticket, make sure the Support Assistant is turned Off as several customers have reported that when the Support Assistant is enabled, the Contact Support option may not appear.

As shown in the screenshot, simply switch Support Assistant > Off, then enter your issue description. Once disabled, the Contact Support button should appear, allowing you to submit the ticket normally.

As a fellow user in this community forum, I do not have access to your organization’s account, backend systems, service logs, or administrative tools. This means I am unable to verify the backend status or make any server-side changes. If this is a backend or service-related issue, Microsoft Support will need to investigate it further.
