A form that was created a little over 5yrs ago has suddenly become unavailable from my work system, it attached to email address [Moderator note: Personally Identifiable Information removed] @united.com.

    I understand that your Microsoft Form has suddenly become unavailable, even though no changes were made.

    To help investigate the issue, could you please provide a bit more information?

    • Are you receiving any error messages, such as "This form is no longer available" or "This form has been deleted"? If so, please share the exact error message or a screenshot.
    • Is the form owned by you, or is it associated with a Microsoft 365 Group or SharePoint site?
    • Has the form been shared with other users as collaborators?
    • If the form was created under your personal account and is not associated with a group, you can check whether it was accidentally deleted by reviewing the Microsoft Forms Recycle Bin here: https://forms.cloud.microsoft/Pages/DesignPageV2.aspx?origin=shell&recyclebin=1
    • If the form is connected to a Microsoft 365 Group or SharePoint site, please check the Recycle Bin of the corresponding SharePoint site, as the form may have been removed there.

    Please give these steps a try and let me know what you find. Any additional details or screenshots would be helpful for further investigation.

    I am the owner, their are 2 collaborators with edit access. I am in contact with 1 of the 2 and its not in their recycle bin. User's image
