Hi Glenn,
May I confirm once again whether the user deleted a form and then he/she also cleared it within the Deleted forms? You can ask your user to go to Microsoft Form > Deleted Forms and double check:
- If the form is still there, the user can select More Options > Restore to recover it
- But if he/she already deleted it from Deleted Forms, that form is permanently deleted, including its responses, and there is no available option to recover even through Microsoft 365 admin recovery.
