Exchange Online Outlook desktop intermittently delays displaying new emails - OWA shows mail correctly

Hi everyone,

I'm hoping someone has seen this before as we've exhausted most of the normal troubleshooting.

Environment

  • Microsoft 365 Exchange Online
  • Outlook for Microsoft 365 (Current Channel)
  • Windows 11
  • Single affected mailbox
  • Cached Mode tested both enabled and disabled

Problem

The user reports that Outlook intermittently does not display newly delivered emails.

Example:

  • User opens Outlook at 09:00.
  • Inbox contains no new mail.
  • User returns later (e.g. 10:30/11:00).
  • Multiple emails suddenly appear in the Inbox.
  • Those emails all have timestamps before 09:00, indicating they should already have been visible.

This has happened multiple times and we have screenshots showing emails appearing later with earlier timestamps.

What we've verified

  • Outlook profile recreated.
  • Brand new Outlook profile tested.
  • Cached Exchange Mode enabled and disabled.
  • Outlook fully updated.
  • Outlook status shows:
    • Connected to Microsoft Exchange
      • All folders are up to date
      • Message Trace confirms Exchange delivered the messages immediately.
      • Internet headers confirm normal delivery.
  • OWA does not appear to show the same issue.
  • Issue was reproduced once on a second Outlook client but has since proved difficult to reproduce consistently.
  • No forwarding configured.
  • No mailbox delegates.
  • Inbox rules checked.
  • Hidden Inbox rules checked via PowerShell.
  • RSS Subscription rules were found but are disabled.
  • RSS Subscriptions folder is empty.
  • Mailbox protocols:
    • MAPI enabled
      • OWA enabled
        • EWS enabled
          • ActiveSync enabled
          • Inbox currently contains approximately:
            • 11,000 items
              • 6.8 GB

Additional information

The mailbox was compromised several months ago and secured. Password reset, MFA enabled, and mailbox rules were reviewed. No evidence of any remaining malicious forwarding or rules.

What we're trying to determine

Has anyone seen Outlook desktop intermittently fail to display newly delivered Inbox messages while:

  • Exchange Message Trace confirms immediate delivery,
  • OWA appears correct,
  • and the issue survives a new Outlook profile?

Are there any known mailbox notification (MAPI) issues or Outlook synchronization problems that would produce this behavior?

Any suggestions would be appreciated.

I understand that you are experiencing an unexpected issue where Outlook desktop intermittently delays displaying new emails, and you have already done a very thorough investigation to isolate this. As a forum moderator, I genuinely wish I could directly inspect the mailbox's MAPI session and service-side diagnostics to fix this for you. Unfortunately, my role here is limited to providing general guidance and solutions that you can apply on your side.

When the issue next occurs, could you record the following before restarting Outlook or changing folders?

  1. If the user presses F9, do the messages appear?
  2. Do they appear after switching to another folder and returning to Inbox?
  3. Do they appear after changing the Density to Compact or running View > Sync?
  4. Are the missing messages visible through Inbox search before they appear in the normal message list?
  5. Is Show Focused Inbox, conversation view, a filter, or Show only unread messages enabled?
  6. Does the issue reproduce with outlook.exe /safe?
  7. When reproduced on the second computer, was it on the same network, VPN, proxy, or security stack?
  8. Are there endpoint security, DLP, email-classification, CRM, or antivirus Outlook add-ins installed?

These distinctions can show whether Outlook has the item but is failing to refresh/render the folder view, or whether the client has not yet received the Exchange folder-change notification. 

Also, have you tested the same mailbox in classic Outlook? If the issue occurs only in new Outlook but not in classic Outlook, that would help isolate it as a new Outlook client-specific synchronization or display issue. If it occurs in both clients, a mailbox, network, or service-specific cause becomes more likely.

At this point, I strongly recommend that the Global Admin in your organization create a service request with Microsoft Support. I would suggest collecting and providing the following:

  • The exact delivery and display times, including timezone
  • The Internet Message ID from message trace
  • Screenshots showing the message in Outlook on the web but absent in new Outlook
  • The new Outlook version
  • Whether manual Sync or changing folders causes it to appear
  • Whether classic Outlook displays the same message immediately 

A technical support engineer can perform a remote session to investigate the situation, verify the backend configurations, and run any necessary synchronization tools to resolve the problem. If the issue requires further attention, they can escalate it to a specialized team for deeper analysis.
