A web-based tool in Microsoft 365 that enables users to quickly create surveys, quizzes, polls, and feedback forms.
For a personal Microsoft 365 (consumer) account, an active Microsoft Form is always owned by the account that created it. If the form is active (still accepting responses) but not visible in the current profile, the most likely cause is that it was created under a different Microsoft account than the one currently signed in.
To locate the form:
- Sign out of all Microsoft accounts in the browser.
- Sign back in with any other Microsoft accounts that might have been used when the form was created (for example, a different personal Microsoft account or an old email alias).
- Open Microsoft Forms and check the list of forms under that account.
If the form belongs to another user (for example, a colleague or former employee), only that owner or a group the form was moved to will see it in their profile. In that case, ownership or access would need to be handled by the current owner or an admin via the standard Microsoft Forms ownership/transfer options.
If the form was owned by an account that has been deleted for more than 30 days, the form can no longer be accessed or transferred. If the owner account is still active and licensed, the form remains tied to that account and cannot be taken over directly from another profile.
