When Windows wakes from a display timeout, a corrupted history cache in the Transient Multimonitor Manager causes it to misread the hardware initialization sequence and invert your monitor layout.
To fix this, first create a backup or System Restore point for safety. Then, open the Registry Editor as an administrator and navigate to HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Control\GraphicsDrivers. Delete only the Configuration, Connectivity, and ScaleFactors subkeys located directly beneath it, leaving the main folder intact.
Restart your computer to force Windows to flush its cache and detect the displays as fresh hardware. Finally, right-click the desktop, open Display Settings, and arrange your monitors correctly.
