Request to Transfer AZ-500 Certification to Personal Account

I earned the AZ-500 certification while working at my previous company, using my corporate email account. About six months ago, I changed jobs, and now when I click on the certificate link, I receive an error message saying the profile cannot be found. How can I transfer or reactivate my certification under my personal Microsoft Learn account?
