Based on my research, this issue is a known limitation of the SharePoint mobile app on Android. Microsoft’s support page for the Forms web part clearly states “The Microsoft Forms web part is not available in SharePoint mobile app”.
In other words, embedding a Form (especially one with file upload fields) on a SharePoint page is not supported on the mobile app

Given this, to work around this issue, I recommend the following approaches:
- Access the SharePoint site and Microsoft Forms using a mobile browser instead of the SharePoint Android app.
- Alternatively, embed the form using the Microsoft Forms web part on the SharePoint page. Note, however, that the Forms web part is not supported in the SharePoint mobile app, so this method is best suited for desktop or browser-based access.
