Hello,
i'm facing an issue in the Teams approval app while sending requests. getting an "unexpected server issue". Other customized templates also do not respond when sending requests. Can anyone suggest what the issue was and how to troubleshoot?

try to turn off the required attachment if you enable it on your template.
it works on me. maybe its a bug .
if still doesnt work, try removing approvers who are not in the organization anymore or relog-in their account on microsoft.

I have the similar issue while making workflow request un-expected error occurred.
