Sadly I also have this issue but on Windows 10.
It never used to be a problem with the same hardware so I can only assume its a change that was applied to both versions.
In my case it is 100% reproducible. It happens when my receiver (effectively my left my left most 'display') that is connected via HDMI to my GPU turns off after having last been turned on.
This shifts everything on my screens over to the right (including the center monitor even though it doesn't get disconnected), disables auto-hide, and displays the prompt about only being able to have one hidden toolbar even though I don't have any toolbars enabled.
