Having the same problem. I've tried all the solutions on the forum and nothing has worked. Talked with Xbox chat support this morning and they said the controller was probably toast but they couldn't help me out due to a 90 day warranty.
Glad to hear Microsoft is continuing with garbage products that they do not support.

So first was to use the included extension usb cable to get the adapter far from any metal that would interfere and to reduce the distance to the controller (it's less than 12" away from the adapter). I then ensured the drivers were
updated, tried updating the controller in the Win 10 Xbox panel under devices, then I went and changed the power settings for the adapter in Device Manager to "not shut down" when increased draw occurs. Nothing has worked.

No I don't have amother controller to test the adapter- though I did use a usb cable to connect the controller directly to a usb 3.0 port and removed the adapter (so it's essentially hard wired) and it still disconnected

So for sure the controller has issues - since it was purchased with the adapter bundle I figure it'd be easier to do a direct swap for the same item
