Page fault in nonpaged area and Memory management BSOD on startup

I have been getting crashes in the last month after the first program opens following startup. The error is most often PAGE_FAULT_IN_NONPAGED_AREA and sometimes MEMORY_MANAGEMENT. The issue usually does not happen twice in a row. That is, when the computer restarts after crashing, it normally runs smoothly until I shut it down again. 

Running memtest for several hours came up clean. chkdsk did not appear to solve anything or reveal any problems. Windows is up to date.

This custom built machine is approx 1.5 years old, problems began 1 month ago. 

EDIT: Crashes just started during "normal" operation instead of simply on startup.

Please run driver verifier (DV) per the instructions in the link below.

Note: Make sure to read and remember the steps to reset DV in the event of a crash, and post a link to the newly generated crash dump here.

    Which software in the list in the link do you have?

    I have Adobe CS6 installed, but I didn't think that came with Lightroom. No other programs look familiar or can be found in my Add/Remove programs list. 

    I am willing to uninstall CS6.

    Here is what you should do. Look in control panel (CP) to see if any of the programs listed in the link below, appear as an entry in CP.

    If you can't find any of them, that means that the program related to, or associated with PxHlpa64.sys may have already been uninstalled, leaving behind, just the driver and possibly registry entries.

    Download "autoruns" and use it to locate and disable the driver, after making certain that all Microsoft entries have been hidden, to eliminate the possibility of a mistake.(instructions in the youtube video link below.)

    DV takes the guess work out of the process when it is successful, and as you can see, PxHlpa64.sys was flagged as the culprit.

    By the way, you should also investigate your NVidia display driver.

    I asked Gerry C J Cornell the same, but what is the preferred method for dealing with this driver? Attempting an automatic update was unsuccessful. Should I uninstall the DVD Device using this driver or simply delete the file "PxHlpa64.sys" manually?

    Thanks for the NVidia info. I'm surprised to see a TDR error. I have yet to notice the warning "display driver has stopped and recovered" on my machine. 

    I've been keeping up to date with NVIDIA driver updates and just did one now. 

    Of note was a dialog box that appeared during "3d vision driver" installation: "Could not start process."

    I don't use the 3d vision capabilities and a search of this error turned up nothing useful.
