EDIT: The problem seems to have been fully resolved by doing a clean install of Windows 10.

I use an AMD A6 processor on my Lenovo IdeaPad Flex 15D. Recently, my PC has been slow, and checking the Task Manager shows that the processor speed is only 0.76-0.78 GHz, while my processor's maximum speed is 2.00 GHz. The Resource Monitor says that my maximum frequency is 38-39% (0.76-0.78 GHz). This happens even when I'm in High Performance mode, and I've checked to make sure that the minimum and maximum speeds (in the power options) are set to 100%. I've also set the ATI Powerplay settings to Maximize Performance. The speed does not increase even when I'm running many programs at once.
Any help would be very appreciated.
I've noticed something interesting... The CPU speed in the Task Manager always seems to hang around 0.7-0.78 GHz, even when I'm not doing anything else on my laptop. Perhaps there's something that is not only preventing it from going above 0.78 GHz, but preventing it from going below 0.7 GHz. I have no idea why this is happening.

I have this same issue on a Dell Inspiron 11 after a Windows 10 upgrade. It happens even when I'm 100% on battery and the adapter is not plugged in. The only thing that seems to fix it (temporarily) is going into hibernation and coming back out.
Simple and cheapest solution which can be tried is to
* Unplug your power adapter and go to Task Manager using Ctrl + Alt + Del command. Go to performance tab and check whether CPU speed has increased or still stuck on 0.78 Ghz. If it has increased then 100% sure that your power adapter needs to be changed.
* Restart your laptop and if it gives a warning of adapter voltage which can be 65W or 90W depend on your hardware.
* Now if you got the error of adapter then simply buy a new adapter with same model and your issue will be resolved.
* Try to check your BIOS version and download a new BIOS version if available on the vendor website.
Main issue is always voltage issue which decrease the speed and performance of hardware.

I had this same problem. Windows 10 automatically put my machine in "balanced" mode and maximum 5% processing speed when I upgraded.
I had a hard time getting it to accept 100% or even high performance mode.... finally it did.
Microsoft needs to stop setting this very hidden parameter of peoples computer....and make sure its working

I have exactly same problem with my Dell Inspiron 13Z (i7 5500U).
Solution was easy... power adapter...
My CPU was limited to 20% of power - switching between 0,3-0,7GHz and everything was damn slow.
When I disconnect are reconnect my power adapter - everything was back in normal. I have no idea why system do this , connector on notebook side was connected wrong but who care? My battery was full...
But - reconnect was solution. :o)
