I had the same problem in Windows 7, and it persists in Windows 8. This happened in three computers I had, including the current one.
I have configured three input languages, and I assign a hotkey (alt+shift+number) to each of them. However, after some time, the hotkeys are reset; sometimes it's just some of them, other times they all disappear. They often keep working, but there are a few times when I can't switch languages in any other than pressing shift+alt or by clicking.
This issue may also be related - Sometimes I can't change languages from IME to English or any other. For that, I have to switch between input methods in IME (with alt+`), and only after I do that I can use the hotkeys. Again, switching languages with shift+alt works, but the custom hotkeys don't.
Does anyone know the cause of the problems I described? Is there a way to fix it? Are there any plans from Microsoft to fix these bugs?

It's been over an year already and the problem persists. It seems like it doesn't go without saying that 3rd party programs and fiddling with register are not solutions - they are workarounds. What we need is a patch from Microsoft. The problem is hotkeys disappearing; the solution is hotkeys not disappearing. Everything else is a workaround, and only Microsoft can fix this definitely.
I won't be following this thread anymore, but I hope to see a useful update in the near future.

I solved that problem by binding Ctrl+Win+Space to CapsLock using AutoHotKey.
Ctrl+Win+Space switch language to last used, so if use mostly ru and eng it will switch beween them. If you need to switch to ja use Win+Space.
Totally useless because of very limited set of supported languages. Also that supported languages are also simple.
Main solution after unification of system language and some languages is off the keyboard shortcut - is program to shift keyboard layout instead of windows one. Like www.arumswitcher.com (it's free).
If some languages in your system have keyboard shortcut and others have not (after reboot or system sleep) - Arum Switcher will switch all of the languages correctly
